Please visit FirstCare's website for online services in order to view plan information, eligibility and claim status information.  You must first register on the website to login.  To register click under “Register Now!”.  Once you have registered, you may login using your Member # (located on your card) as your login and your password.

BlueCross BlueShield - www.bcbstx.com

Please visit BlueCross BlueShield's website for online services in order to view plan information, eligibility and claim status information.  You must first register on the website to login. 

Claim Forms

Click here to view the FirstCare (medical, vision, dental), BlueCross BlueShield (medical and dental) and Boon-Chapman (flexible spending account) claim forms.  All claims for expenses incurred in 2008 must be filed using the 2008 claim forms.

Please visit the Express Scripts website in order to view information concerning your prescription plan.  Before getting started you must activate your account by clicking on the link entitled “Activate your Account”.  After setting up a username and password, you will be able to view plan information including the Preferred Drug Listing and pricing information.

Summary Annual Report (2006)

The Summary Annual Report gives a detailed description of the Plan expenses and investments for the prior year.
