Retirement Plan

If you were hired prior to March 31, 2006, are at least 21 years of age, have been employed with United for five years, and have completed 1000 hours in at least one of those five years, you are eligible to participate in the Retirement Plan.

Beneficiary Form

Click here to download the Beneficiary Form. As a participant in the retirement plan, you must have a beneficiary designation in the event that anything should happen to you. A participant who does not make a beneficiary designation and is eligible to receive a benefit will have their account sent to their estate upon their death.

Spousal Consent Form

If you are married and do not wish to have your spouse designated as your beneficiary, you must have your spouse sign the Spousal Consent Form. The form must also be notarized by a licensed Notary Public.
